Administrative Hearing Request.
This link contains information about how to request a Washington State Department of Licensing Hearing, also called an "Implied Consent" DOL hearing, after a person has been arrested for DUI and/or Physical Control.  Please note that you should speak with an attorney immediately after being stopped for an alcohol related driving offense so that he or she can advise you on the law as there are limitations on the period of time that you have to request this hearing.
